documents needed for enrollment
All applicants must provide the following Documents:
- Copy of Birth Certificate
- Recent Report Cards/Standardized Test Scores (for grades 1st-12th)
- Pastor Reference Form
- Grammar School Reference Form  For incoming 1st-6th grade-reference not required for kindergarten
- Upper School Reference Form  For incoming 7th-12th grade
*For incoming students from homeschool, please provide a list of curriculum used and/or the documentation you provide to the state each school yearÂ
